How behavior is categorized, who responds, and what gets documented at each level.

1Level 1 · Minimally Disruptive

Behavior examples

Off-task behavior, talking out of turn, minor classroom disruption, inappropriate noises, refusal to follow minor directions, unprepared for class, minor horseplay, misuse of materials.

Staff actions / consequences

Teacher-managed. Redirect, proximity control, reteach expectations, warning, seat change, behavior reflection, parent contact as needed.

Documentation

No formal documentation required. Teacher may keep personal behavior notes if patterns develop.

2Level 2 · Somewhat to Significant Disruption

Behavior examples

Repeated Level 1 behaviors, defiance, disrespect toward peers or staff, classroom disruption affecting learning, inappropriate language, leaving assigned area without permission, minor dishonesty, technology misuse, repeated refusal to work.

Staff actions / consequences

Teacher intervention plus parent contact. Student conference, behavior reflection, loss of privilege, buddy classroom, restorative conversation. Administration may be notified if behavior continues.

Documentation

Teacher documents incident in behavior tracking system and records parent contact.

3Level 3 · Significant Disruption

Behavior examples

Threats, bullying, harassment, repeated defiance, profanity directed at others, fighting without injury, vandalism, theft, repeated classroom removals, major disruption preventing instruction, inappropriate physical contact, leaving campus/building without permission.

Staff actions / consequences

Administrative referral required. Student removed from setting. Parent contact by administration. Restorative conference, behavior plan, detention, in-school suspension, or other disciplinary action.

Documentation

Office referral completed. Administration documents investigation, parent contact, and consequences.

4Level 4 · Emergency — Harm to Self/Others

Behavior examples

Physical assault causing injury, credible threats, possession of dangerous weapon, self-harm behaviors or threats, severe aggression, major fight, destruction creating safety risk, behavior requiring immediate crisis response.

Staff actions / consequences

Immediate administrative involvement. Secure safety of students and staff. Contact administration, counselor, SRO/law enforcement (when appropriate), and parents. Crisis intervention procedures followed.

Documentation

Formal incident report required. Administrative documentation, parent notification, safety/crisis documentation, and district reporting as required.
